In order to increase access to justice and ensure compliance with social distancing during the COVID-19 public health emergency, the notary requirement has been temporarily removed from the forms and replaced with this language:

The information above is true, complete, and accurate to the best of my knowledge. I understand that knowingly providing false information in this document may result in a contempt of court finding against me which could result in a jail sentence and fine, or criminal penalties under R.C. 2921.13.

This change will ease the difficulty parties are having getting essential court documents notarized.  Due to Ohio’s State of Emergency order requiring social distancing, banks, clerk’s offices, and attorneys, where parties often sought notaries, have reduced or eliminated those services.

These forms will be accepted by local courts.
